  1. Check to see if you meet the criteria that is required to register for medical marijuana certificate through iMed Chiropractic. Note that the following are conditions/symptoms that iMed Chiropractic in Fairfield would gladly diagnose and treat in-house and do not require a furnished note from an outside physician.*However, if you are a Medicare recipient you will need to obtain a note from your treating physician stating you have been diagnosed with one of the conditions listed immediately below.
    • Widespread body aches, fatigue, and tenderness (Fibromyalgia)
    • Headaches that are difficult to treat, or that fail to respond to standard treatment (Intractable Headaches/Migraines/TMJ)
    • Degenerative Osteoarthritis and associated spinal disorders that may cause nerve pain (Sciatica)
    • Chronic Neuropathic Pain (Nerve Pain)
    • Failed Back or Neck Surgery Syndrome with Persisting Pain/Post Laminectomy Syndrome
  2. Obtain a note (including diagnosis and letter head) from your treating physician if you have been diagnosed with one of the following:
    • Muscular Dystrophy
    • Multiple Sclerosis
    • ALS
    • Osteogenesis Imperfecta
    • Post Herpetic Neuralgia
    • Intractable Seizure Disorder*
    • Irreversible Spinal Cord Injury with Intractable Spasticity
    • Cystic Fibrosis
    • Cerebral Palsy
    • Ulcerative Colitis
    • Crohn's Disease
    • Epilepsy
    • Parkinson's Disease
    • Positive Status for Human Immunodeficiency Virus or Acquired Immune Deficiency Syndrome
    • Psoriatic Arthritis
    • Complex Regional Pain Syndrome
    • Rheumatoid Arthritis
    • Sickle Cell Disease
    • Neuropathic Facial Pain
    • Hydrocephalus with Intractable Headache
    • PTSD
  3. Complete our registration form iMed Chiropractic offers our patient form(s) online so they can be completed in the convenience of your own home or office. BE SURE TO INCLUDE A VALID EMAIL ADDRESS. (This email address will be used to sign up with the State of Connecticut Department of Consumer Protection).
  4. Submit any pertinent health records via email: info@imedchiro or fax: (203) 366-7588, or bring them during your visit.
    • Note on letter head from your treating physician stating your diagnosis.
    • Any imaging reports including MRI or X-ray
    • Please include a copy, front and back of your driver's license and health insurance.
  5. Call iMed Chiropractic at 203-333-7788 to schedule your Medical Marijuana Certification Exam.
  6. Provide a Connecticut ID and payment due in the form of cash or credit card prior to the exam. (Examinations are $160 and annual renewals are $110.)
  7. Once the certification exam is complete you will receive an email from the Connecticut Department of Consumer Protection (DPC) confirming that an iMed Chiropractic medical provider has recommended you into the program. You then have to create a "DAS Business Network Account" through the CT Department of Consumer Protection.
    1. Click the link (https://www.biznet.ct.gov/AccountMaint/Login.aspx)
    2. Click the "Log In" button in the top left corner.
    3. Log in to your account with the email address you provided iMed Chiropractic during your certification exam.
    4. Verify that the information submitted is accurate and answer a series of certification questions.
    5. You need to submit the following to the DCP:
      • Proof of Identity ( see examples )
      • Proof of Connecticut Residency ( see examples )
      • Current passport sized photograph ( see photograph requirements )
      • $100.00 registration fee (checks/money orders should be made payable to "Treasurer, State of CT")
      • You will be able to upload these documents and pay the fee when you submit your registration application online
  8. Allow 4 weeks to process.
